Now you can register several days before your appointment using your smartphone, tablet or computer.
That’s right, no more paper forms to fill out! You will save time at the front desk by filling out demographics and screening forms at your convenience, save paper, and be able to focus on getting the most out of your time with your provider whether that’s comforting a sick child or ensuring all of your questions are answered at well checks.
 Here’s how it works:
- 3 days before your appointment, you’ll receive a text or email with a secure link to register
- Follow the instructions for providing the information we need for your upcoming appointment
- You’ll also have the chance to update your medical information and sign any necessary forms.
Make sure that we have an updated phone number when you schedule your appointment. But don’t worry, we have dedicated tablets to use at the office if there are technology barriers for you at home! |
